Car/driver certification

The following list of paperwork required to get certified to drive Brianna says is on the athletic webpage but apparently not in the paperwork package. If you are still in the process of working on this the total list foolows.

Here is a list of what we require to be a volunteer driver:

a) Form CUSD #217 - Classroom Volunteer and Field Trip Chaperone or Driver Application

b) Form CUSD #212 A - Vehicle Safety Certification (cost $12 at Lemos 76)

    Form CUSD #212 B - Auto Service Record

c) DMV Driving Record

1) Log on to www.dmv.ca.gov

2) Complete request for driving record (cost $2.00)

3) Print out and submit to school office along with other forms

Also provide the District:

d) A copy of your valid driver's license

e) A copy of your current vehicle registration

f ) A copy of your valid vehicle insurance card
